Center TRT Evaluation Materials
For new adopters wishing to implement and evaluate an intervention similar to this one, Center TRT has developed an evaluation logic model and an evaluation plan. The logic model is intended to guide the evaluation process (as opposed to the planning process); the evaluation plan focuses on the implementation and effectiveness of a similar program. This evaluation plan focuses on determining reach, adoption, and the extent of program implementation, as well as the effectiveness of a QRIS in addressing targeted outcomes. The evaluation is a pre-post design with no comparison group. This evaluation plan provides guidance on evaluation questions and types and sources of data for both process and outcome evaluation. If you are interested in answering evaluation questions not listed in the evaluation plan, please refer to the list of additional evaluation questions here. We suggest a variety of data collection tools throughout the evaluation plan.
Evaluation Materials developed by South Carolina
The assessment tools below were created by South Carolina ABC Quality program staff and are available for your review and use. These tools have been incorporated into the evaluation plan that Center TRT developed. Please note that Center TRT has not reviewed these tools. Please provide credit to the South Carolina Department of Social Services and Division of Health and Environmental Control when reproducing materials in the original, or adapted, form.
- Level A assessment tool – mandatory standards – This is the assessment tool that South Carolina developed to assess the mandatory standards for Level A centers.
Level C assessment tool – This is the South Carolina-developed Level C assessment tool. Changes for Grow Healthy are not highlighted in this tool.
